Office Hours:  Regular front desk hours are Monday through Thursday 8:00 a.m. to 4:30 p.m. and Friday from 8:00 a.m. to 12:00 p.m.

INSURANCE:

​

Our office is not contracted with any insurance companies and as a courtesy, our office will submit claims to your insurance for any out-of-network benefits that may be available to you.  Effective January 1, 2022, you may request a "Good Faith Estimate" Under the No Surprise Act.  Please contact our office for additional information.

* Court Ordered Evaluations or Custody Evaluations

* Workers Comp Evaluations

* Disability Evaluations
